Our Access, Advice and Information Team is your first contact for social care support (excluding transition referrals). We are dedicated to helping you get the right support, at the right time.
What We Do
• Receive Referrals: We handle all new referrals for adult social care, ensuring your enquiry is directed appropriately.
• Initial Assessments: Our team carries out building on strengths conversations—an initial assessment to help us understand your needs.
• Interim Support: If needed, we can commission interim packages of care to ensure support is in place quickly.
• Equipment & Technology: We offer simple equipment and consider technology solutions to assist you in maintaining your independence.
• Connecting You: We refer to external agencies such as the Community Wellbeing Hub, making sure you can access the full range of support available.
• Next Steps: If a more detailed assessment is needed for long-term support, we transfer your case to the most appropriate Adult Social Work Service or Occupational Therapy Team, including arranging appointments at our OT Assessment Centre.
