This form is for professionals who are working with B&NES residents with a long term disability which directly affects their ability to manage at home.

We are not able to help if:

• The person has a short-term health condition which is expected to improve
• The person has recently been discharged from hospital (Click here to access the Care Coordination Centre).
• The person requires rehabilitation to help carry out daily living activities - for example, to make meals, get dressed, or return to previous level of function (Click here to access the Care Coordination Centre).
• The person needs equipment for employment or education (Click here for Access to Work)
