A review of the current authorisation can be requested if:
- The person no longer meets the criteria for DoLS or the reason why they meet the criteria has changed
AND/OR
- Conditions attached to the authorisation need to be varied due to a change in the person's circumstances.
B&NES DoLs Team process:
You will receive a submitted form notification from this portal to your registered email address confirming we have received your Form 10.
DoLs Team will triage each referral and be in contact with you to discuss.
